| *Membership Pricing with 1 year signed contract $39/month for a single member and $49/month for the whole family. Monthly memberships will be automatically deducted from debit or credit card each month.
Other Discounts: For members who pay for the year upfront, a one month discount is applied (pay for 11 months and get 12). At the end of the yearly membership, the account will be charged on a month to month basis unless another year is paid upfront. Multiple pre-paid visits qualify for 5-10% discount. Cancellation Policy: Please notify our office if you cannot make your appointment 24 hours prior to the appointment time. “No Show” and same-day cancellations will be subject to 50% of the office visit fee scheduled. We understand life happens and emergencies are not subject to this policy. Payment for services is due at the time that services are rendered. Payments may be made in cash, check, debit card, MC, Visa, Discover, AE, or with an HSA card. There are no refunds for memberships or pre-paid visit packages. |
